Residentially, the lease itself is the standing obligation and many landlords never invoice. But tenants who need expense documentation appreciate one. Commercially, monthly invoices itemizing base rent and pass-through charges are the norm and often the lease's requirement.

What are CAM charges and how do they bill?

Common-area maintenance. The tenant's share of shared-space costs under NNN-style leases. They bill monthly as estimates with an annual true-up; both the estimate lines and the reconciliation belong on invoices the tenant can audit against the lease's definitions.

How should late fees be stated?

Exactly as the lease states them, within your state's limits on amount and grace period, and on the invoice as a term, not a surprise charge. A late fee that appears without a stated basis is the easiest charge for a tenant to refuse.
